Do not try to fill out this form online—it won't happen! Deliveries are made to the East Bay on Tuesdays, Wednesdays, and Fridays. Members pay in advance for either one month, three months, or one year. If you want to pay monthly, send $72 to start ($18 x 4 weeks). If you want to pay quarterly, send $214.50 to start ($16.50 x 13 weeks). With home delivery, add $7 per week, or pay $100 for 4 weeks ($25 x 4 weeks), $305.50 for 13 weeks ($23.50 x 13 weeks). You can also request every other week deliveries, however, you still need to pay for 13 boxes to get the quarterly discount rate.
