Do not try to fill out this form online—it won't happen! Deliveries
are made to the East Bay on Tuesdays, Wednesdays, and Fridays. Members
pay in advance for either one month, three months, or one year. If you
want to pay monthly, send $68 to start ($17 x 4 weeks). If you want to
pay quarterly, send $214.50 to start ($16.50 x 13 weeks). With home delivery, add $7 per week, or pay $96 for 4 weeks ($24 x 4 weeks), $305.50 for 13 weeks ($23.50 x 13 weeks). We will e-mail
a confirmation and orientation packet when we process your application.
